A behavioral health hospital in the Florida Panhandle is hiring a Case Manager RN. This is a placement managed by Lela Boom Consulting Group.
About the role
The RN Case Manager Care Coordinator facilitates the interdisciplinary plan of care with a focus on evaluating the appropriateness of clinical care, medical necessity, admission status, level of care, and resource management. In this position, you will conduct comprehensive psychosocial and medical assessments, develop and document individualized case management plans of care, and lead interdisciplinary team meetings to ensure collaboration across clinical disciplines. You will evaluate admissions for medical necessity using approved criteria, perform utilization management reviews, and communicate findings to third-party payors. A key focus of the role is identifying barriers to patient throughput and quality outcomes, tracking and trending variances, and developing action plans to drive process improvements. You will also serve as a liaison between physicians, patients and families, hospital staff, and outside agencies, facilitating appropriate discharge planning and connecting patients — including those in underserved populations — with community-based resources and support programs.
What you need
- Minimum of 3 years of recent (within the last year) Case Manager experience in an acute care setting; candidates with 3 years of experience on Med/Surg, Tele, Neuro, ICU, PCU, or ED units will also be considered; candidates with Case Manager experience in home health or insurance will be considered if they also have 3 years of acute care experience total and at least 1 year of acute care experience within the last 5 years
- Associates Degree in Nursing or Diploma in Nursing required
- Bachelor's Degree in Nursing preferred
- Current FL RN license required or appropriate compact licensure; if compact license held, active FL RN license required within 90 days of hire
- Advanced Practice Registered Nurse license is acceptable for position if current and compliant
- Certification in Case Management, Nursing, or Utilization Review preferred
Schedule
Day shift, 8:00 AM to 4:30 PM EST, with one weekend per four-week schedule.
Pay and benefits
$31.94 - $43.92/hr plus $10,000 sign-on bonus
